** We can file on any dental insurance company that allows you to see any dentist. Please bring your dental insurance card for your visit (if copy need front & back) will need name of dental insurance carrier, group number, subscriber ID (w/birthdate & social security number of subscriber) 800 phone number, address to mail claims, some insurance companies have over 50 locations, that way we can locate yours.
